A form is a page on the World Wide Web that offers fields you can fill out and send in, such as an on-line order form. They are used on the Internet to allow users to enter requested data, such as adding a name to a mailing list or ordering software. Like a paper form, fields may include fill-in text boxes in which the user types in a name and an e-mail address. Check boxes allow you to check whichever choices apply. Radio buttons are like check boxes, except that you may choose only one. A list box gives users the opportunity to choose one of the possibilities in the box.
